Request Assistance

Only members of the Gaspé Cancer Foundation can request assistance. Membership must be in good standing for the current year.

To make a claim, a member must have been referred to a facility outside the region by a health professional.

Download and print our referral form. Have your doctor complete the first part and the treating facility complete the second, then send it to us by mail or in person at the office on a Friday.
